By submitting this registration form to the stallion sports club, I am registering to participate in the indoor and outdoor activities organized by the club. I agree not to make any claim and to hold the club, its officers and directors in no way responsible for any incident that could cause loss, damage or bodily injury caused by direct or indirect participation in said activities. As a registered member, I acknowledge that the jersey is on loan from the stallion sports club, and I agree to return it to the club upon request. Do not return the jersey or put it back in bad condition, this will incur an additional charge of $75.00. These compensation costs for the lost or damaged jersey are payable to the club. I have read the instructions and regulations established for the practice of volleyball or basketball, issued by the federal, provincial and municipal authorities as well as those stipulated by the administration of the Stallion Sports Club.
